At Jerry Yang, DDS, we strive to provide you with the highest quality dental care. As a courtesy, we handle the completion and filing of insurance claims for your treatment. However, please be aware that the services we recommend and perform are based solely on what’s best for your oral health—not on the specifics of your insurance policy or the benefit package it offers.
Changes in Insurance
To avoid any delays in reimbursement, it is crucial that we have your current insurance information on file and that you inform us promptly of any changes to your plan. Outdated information can result in additional delays and may require immediate payment from you, as the balance is ultimately your responsibility.
Please remember that dental insurance was not designed to pay for all dental care. Most contracts have limits and/or various degrees of co-payment, which may affect your coverage. Insurance payments, including “usual and customary” rates, are based on premiums paid to your carrier and do not reflect our actual fees, which are determined by the cost of materials, time spent, and our commitment to delivering the highest quality care. This is not determined by your insurance carrier or their benefit package, but by our practice’s dedication to providing you with the best possible care.
Important Note
It’s important to understand that insurance carriers do not release information to third parties, including our dental practice, even if we have signed consent on file. While we make every effort to inform you of your coverage limits prior to procedures and treatment, please keep in mind that insurance benefits can have many limitations and stipulations. This is why we provide only an estimate of coverage and not a guarantee, as we cannot foresee all possible limitations imposed by your policy.
Reminder
We allow 30 days for your insurance company to reimburse our office. After this period, any outstanding balances or further inquiries with your insurance provider will become your responsibility.
