Important Information for
Patients With Insurance
Dear Valued Patient,
At Jerry Yang, DDS, we strive to provide you with the best possible dental care. As a courtesy, we handle the completion and filing of insurance claims for your treatment. However, it’s important to note that the services we recommend and perform are based on what’s best for your oral health—not on the specifics of your insurance policy.
Changes in Insurance: It is crucial that we have your current insurance information on file and that you inform us promptly of any changes to your plan. Delays in reimbursement caused by outdated information can result in delays and additional work. As your balance is ultimately your responsibility, we will request immediate payment if your insurance company is delayed due to misinformation.
Please keep in mind that dental insurance is not designed to cover all expenses. Most plans come with limits and co-payment requirements. Insurance payments, including “usual and customary” rates, are dictated by the premiums paid and do not reflect our actual fees. Our fees are determined by the cost of materials, the time spent, and our commitment to providing you with the highest quality care.
Important Reminder: We allow 30 days for your insurance company to reimburse our office. After this period, any further inquiries with your insurance provider or outstanding balances will become your responsibility.
Thank you for your understanding and cooperation.
